Either your bank was totally misinformed or you misheard. When you use a Debit/Credit Card to purchase something in another COUNTRY you may be subject to an International Currency Conversion Fee(or similar name). With the Internet it is sometimes hard to tell because it does not matter where the company is located but the bank they process. A lot of companies in the US actually process web payments out of Canada. The fee can vary but a 1% fee(which this calculates to), is in the 1%-4% usual range. However, when I have seen this the bank separates out this fee from the actual purchase.

There are also cases where you may be subject to that states Sales Tax, such as if you are in NV and buy something from NY the company may charge you NY sales tax. Although this case seems a little low for this.

A third possibility is that you didn't read the cost correctly and mixed up $4.95 and $4.99.

I am 45, and granted, new to ordering online but I have never heard of anything other than you CAN be charged your state sales tax on it. Which I doubt is the case cuz it's only one percent. Furthermore, the Truth in Lending Act may cover this: if there are additional fees, they must be disclosed to the buyer (you) prior to purchase. to not state fees but later charge them is illegal.

Now that is BS. I would transfer my money to a different bank immediately. I've never heard of such a thing. Different country and money conversions yes but not a different state. Dump them, put your money in a bank that doesn't nickle and dime you. I am curious as to what bank this is.

That is total crap. You need to march down to the bank and demand to talk to somebody about that because charging you fees for using your card across state lines is bogus.
