Appropriate ID for a bank account?

I am 16 and need a bank account to receive EMA next year but I don't have a passport as proof of ID the only photographic ID I have is school smart card from a school I no longer attend. Will the smart card be sufficient if not what will?

How can I obtain proof of address?

As the ID is for name and address, I suggest taking the letter offering the EMA as some proof of ID.

As you are under 18, you may also need the signature of a parent or guardian who will need to take evidence of their own ID eg Driving Licence, utility bill council tax bill etc.
