personally, i used northwest (they ship nationwide) http://nwcryobank.com/ and will ship to your home, where you can do home inseminations... which are more personal. they will also ship to a doctors offics or clinic if you choose to go that route - most banks will ship directly to the clinic.
first though, if you are using a doctor/clinic, you should find out if they have a bank preference. some clinics only work with a specific bank, or they will give you a few to choose from. others just want you to supply the sperm and they'll do it.
if you're doing it the "at home" way, northwest is the only bank ive found that will ship to your home without needing a doctors signature. :)
