It's part of Health Club in Layton, Utah. (Simply Googling will show that there is a Layton and no Clayton, Utah.)
Sounds like your card was stolen since you don't know the name. See this complaint site. They apparently have a history of consumer complaints.
Just a question (to jog* your memory): Did you join a gym recently? Is it possible that this is the parent company of the gym you joined?

definitely do contact your bank.
Your account has been hacked. Aside from that, I have many friends who have had major problems with that bank and would never have anything to do with them in asny way again. You Mary wasn't to consider that.
