I know this will sound silly but, have you changed your last name and address on your legal ID or driver's license? I can understand a bank not taking a birth certificate because in order to have your married name changed back to your maiden name it has to be in the official degree signed by the judge. I had to have the decree with the seal before the DMV would allow me to change my last name and then I had a legal ID to present for things like bank accounts, address changes and such.
I'd take it up with a lawyer and sue. With sufficient legal backing they'll bow to your demands. As it is they are in alot of hot water for illegal activities regarding their clients.
